At Sullivan Physiotherapy Clinic, we offer direct billing for most insurance companies. If online direct billing is not available, you will be asked to pay for your visit up front and we will provide you with a receipt which you can submit to your extended health provider for reimbursement.

You can call to your extended health carrier and ask about your coverage limit and percentage.

You need to bring

• Your extended benefits card with policy number and member ID.
• Doctor’s referral, if required by your extended health insurance policy.
